How can I add a booking app with preset mileage surcharges on Shopify?

I need a booking app for my shopify site where i can add preset mileage surcharges and let customer pick from various service products.

I'd then need to approve the date, time and location before approving customer to checkout.

I had a look at a couple to see if they would suit your specific needs, and I can see that Easy Appointment Booking states that it offers: 'variants to control the schedule by instructor, service, or duration on a single product'.

This may allow you to account for mileage in the way you requested, though in order to be entirely certain I'd recommend you reach out to the app developers who will be best positioned to answer any questions you have about their product. 

It also has a number of good reviews and offers a free plan as well as integrations with calendar services, etc.

Spend some time looking at your options there in the store and reach out to a few apps to see if they can be used in the way you desire.
