How can I change the cart page thumbnail based on user action?

Hello, i've seen an app that can change our cart page thumbnail based on user action. So i build a customizer, i want to display user result as the product thumbnail on cart and checkout, how can i do that from app?

so either i create a custom cart for my app or just use placeholder for the thumbnail that represent the product?
