How can I display form data in a table format on my website?

I am looking for an app, feature, code or something that would enable me to use Shopify features as normal, but also have the ability within my site to:

1- Customer fills out a form on a page (customized form)

2- On a different page or an embedded section etc etc, some of the data from the form appears in a list/table format

3- Need to be able to filter and change sort features of the data as well

This might mean I need two APP's one to collect the form data and another to display the data. The only option i have found so far is APP options that allow you to upload CSV to them and then display the data in table format, i need this to be real-time and live. 

The best example I can give to explain what it is I am trying to do is, the customer fills out a form that says their event is on this date, all the details, etc and then on an events page all those "form entries" are listed, with the ability to search and filter.

We have an app which allows you to create a live form, collect visitors' email address, see the list of the people who subscribed, filter and exported the list. You can check it out here.

This is useful for the first element of my problem, but then how can I show the "form answers" on my webpage. 

Need a way of extracting the data from the data sample into a web format.

The information of your subscribers will be shown in our app "Audience" section. I understand you want to show the data on your webpage, but it's not possible with our apps or other Email marketing app. Customers data is consider private and will show only in the app and your Shopify back end. 

We do allow exporting contact list as csv file.
