There is a free app called Upsell Recommendations that I think will suit your needs. They allow you to inject product recommendations on various pages of your store, including the cart page.

Is there a particular reason that you would like to offer upsells on the cart page? Are you trying to increase total order amounts?

I need to be able to select a specific product that will be recommended. I also need to be able to customise text and headline. Can this be done with these apps?

You can either add a collection, select the products you want via theme settings or modify the product recommendations. https://shopify.dev/themes/product-merchandising/recommendations/show-product-recommendations

Most wouldn't be too hard. You don't need an app.
