How can I fix my clothing size app issue on debut theme?

Hi everyone, i am new here. not a wiz but did set up my own store on debut theme on my own. used free app for the clothing sizes but few months back this is what is see on my website. Went to the backend end dont know where to upgrade or to fix it.
