How can I manage a pop-up for sending physical gift cards?

Hi there. 

Can someone help me? I'm trying to find the app where I'll be able to manage the pop-up for sending the gift card.

For example - I have a physical gift card and I need that my client will be able to insert the Greeting and recipient name. And that's all. No need for a recipient email or other points. And In the order details, I'll see that user wants that this card will send to the shipping address with a Greeting
