How can I set up a UX similar to oatbox.com for subscription flow?

Hi All,

I am wondering if someone can tell me what subscription module(s)/app(s) can be used to create a UX such as that on www.oatbox.com?  Basically, all the subscription apps I have seen (i.e. Bold or Recharge) are robust, but work at the single item level - however oatbox.com offers the subscription frequency on the same page as where the user can choose between multiple items.  See attached for reference screenshot, or visit oatbox.com to see for yourself.  I am building a similar e-comm store and would like for my subscriptions to work in a similar fashion at oatbox.  Thanks in advance!
