How do I create a special product area on my webpage?

Hello.
I cannot create a special area for the name to be written specifically for the product I want to sell on my website. Which application should I use to make it like in the photo I will share as an example. Which ones can you recommend for free?

I also need to request some information specifically for another product and I need to request a photo. I will share a sample photo for this. I would like you to recommend an add-on application that will add text and photos.
