How do I put my Postscripts SMS opt in box on my Shopify password Page?

I am currently having trouble placing my Postscripts SMS opt in box on my shopify password page. It's allowing me to place the opt in box on it's own separate page but not on the password page. Is it my shopify theme that's the problem or is there an extra step I need to take in this case?
