How to add a 'book club' subscription service to online shop

Hi, I want to add a subscription service to a shop which will act as a book club, delivering 1 book a month. This will sit separately to the rest of the shop - and needs to include the following:

• Option to collect information on age of child, gender of child, and box to include interests (so to match books)
• Option to choose a 3, 6 or 12 month subscription 

I was hoping for some help on how to do this?

• What is the best app for this?
• If we add an app, will this allow us to set up the subscription service separate to the rest of the online store (I don't want all the products to be available as a subscription)?
• Do you set up the 3, 6 and 12 month subscriptions as separate products? What is the best way to approach this?
• Also, I understand that there are limitations here in terms of Shopify payments, and no local delivery/pick-up. Is that right?
