How to add custom fields for Transit Plus ID and Client in checkout process?

I am looking to open a store that sells transit tickets. One of the tickets sold at a reduced priced by our paratransit team. In order to qualify for the reduced pricing you must be verified as eligible first in person. Once you have been verified you are given an ID number that matches to your name. 

I need to be able to have two custom fields somewhere in the check out process to collect the information Transit Plus Client and Transit Plus ID Number.

Does anyone know of an app that includes this feature? I have not been able to find a suitable option.

Integrating extra fields into the Checkout is viable exclusively for Shopify Plus accounts. Alternatively, I recommend incorporating the additional fields on the cart page to collect the necessary data.

Consider adding custom fields (Transit Plus Client and Transit Plus ID Number) directly to your product page, ensuring customers fill them before adding the product to their cart. You can achieve this using the Free Easify Product Options app 🤗:

• Create a new option set

• Add a Text Box option for Transit Plus Client:

• Add another Text Box (or Number Text Box) option for Transit Plus ID Number:

• Assign these custom fields to the relevant products:
