How to get customer form data into shopify backend

Hello,

I have an issue in ecook book customer form.

When customer fill up this ecook book form then click on submit button, customer redirect thank you page but the customer form data not stored into shopify backend like "Customers" list.

Anyone help me setup this form and customer data get into shopify backend customer list.
