I am looking for a specific tier discount app

I am looking for an app who can display a Tier discount like in the image.

I have tried many app, but no app comes close to this solution. 

Including:

- preselected quantity with the total price

- the discounted price per item 

- the total amount saved

- Showing the lowest price in order to show the highest % of discount

- per product and/or collection

Who can help me out!
