Is it possible to create a custom page post-checkout without extensions?

Hi, is it possible to create new page after checkout and before thank you page without using post-purchase checkout extension? (Since it has many limitations) 

The new page will not be about offering product upsells. It is a prescription page where customer uploads file and input necessary prescription information.

Since post-purchase checkout UI extension has a limitation when it comes to DOM manipulation, using own css, js and html tags, is it possible with creating shopify custom app?

There is a possibility to add post-purchase content and/or survey (that somehow looks like what you need to).

To tell the story short - your task can be solved by a Shopify developer or agency with a middle level of custom development proficiency.
