Is there an app that allows customers to upload files which will then be sent to me?

I want to create a sign up form which will allow the visitors to already upload their documents before hitting Send. 

Is there one that exists or do you know of any other way I can make that possible?

Sure, look for "custom registration form" apps in the Shopify App Store. Some of these apps will allow you to create precisely that, custom registration forms with custom fields like an "upload file" field.
