Liquid Code for Shopify Forms

Hi guys,

I'm trying to figure out what the code is to get a shopify forms form into my website WITHOUT using the app section. 

I'm trying to build a 'notify me' button on out of stock products, and want this form to show up. This isnt so hard, but I can't simply find the relevant code to add my form, lol.
