Looking for a responsive form building app with Shopify integration?

I am looking to build a responsive Form to capture client data. The app itself allowing me to do this must have the ability for their to be responsive questions. 

i.e. if you click yes, then a further question opens up for example. 

In addition I would like the opportunity for the form to have question marks, so i can provide an explanation to the customer of what the certain question means. 

I can see basic form builders online, that have 6 boxes and contact page for example. These will not work

Does anyone know of a form building app that integrates with Shopify and will provide me with the functionality I require?
