Looking for a review app with fixed options that rate specific features on a scale

Hi, due to a local law we can't allow people to say certain things about our supplements. I want to counter this by giving people the ability to rate some specific features on a scale of 1 to 5 starts. Which review tool can do this? Thanks a lot!
