Looking for a way to capture video testimonials

I'm looking for an app that will help me capture video testimonials or reviews from customers.

Most of the apps I've come across only seem to support text and photos.

Any suggestions are welcome!

I would recommend looking into Stamped.io Product Reviews for an app with video review capabilities.  They support both photo and video reviews. Additionally, you can even offer coupons to reviewers. This is a great way to incentivize them to take a photo or video review.
