Looking for an app recommendation - mix between quiz app and product customization app

Hello!

I am looking for an app that is an amalgamation of a quiz app and a product customization/building app. I want it to look and work like a quiz, but I want to be able to add in features like check boxes, drop down lists, text boxes and live preview for customers to customize a product. 

Can someone please point me in the right direction of an app that can do this? Or is this something that typically requires hiring an expert that can do coding?
