Need suggestions for an order form app with buyer input text?

Hi,

Looking for a app that can provide me with a order form that is buyer input text (don't need drop downs etc).

Also need the option to add a new line item which will increase the price by x amount.

So for each item they add it will increase the price. 

Any questions or anyone who can program this for me let me know.
