Offering A Delivery Subscription But Deliver To Multiple Addresses

I'm working with a client who has a flower delivery service. What they want is to offer a subscription that can be delivered to different addresses but under the umbrella of one order/subscription 

Anyone know an app that is capable of doing this?

This is more of a frontend customization and backend automation creating/editing ordersthan it is an off the shelf app.

Most apps have a narrow scope, a subscription app isn't typically going to have advanced shipping/logistics behaviors.

A frontend customization for gathering the additional addresses.

And an automation app like mechanic to manage orders https://tasks.mechanic.dev/demonstration-order-editing 

And or an email/forms app to gathering the additional addresses after the main order is placed.

That is if this isn't just for customers needing to be able to fix the delivery address, or the initial purchaser can't simply fill in the billing and shipping info as separate addresses in the checkout itself for some reason.
