Online Pharmacy Customisations

hi there

totally new to the forum and shopify. im setting up an online drugstore/ pharmacy. prior to purchasing a medication i require the customer/ patient to complete a questionnaire (medical history, suitability of treatment etc)

i require the website user to complete questionnaire once product selected and prior to checkout. 

each product requires its own unique questionnaire. 

can anyone help re which app to use/ how to go about this. its basically function similar to magento prescription question plugin

Hello, there!

Karlie here from the Shopify Guru Team! There are a few different ways that you can collect information from your customers on your storefront. I'm happy to share some options with you that I think could provide you with the functionality you're looking for. 

Your first option would be to look into using cart attributes that you can enable from within your Shopify Admin. Cart attributes would allow you to collect information from your customers on your storefront cart page of the customer's order stage. You can enable this feature by adjusting your theme coding. This is an advanced guide, but if you're interested in following our doc then you can check it out here.

Another option would be to use an app that would allow you to create a form, survey or poll that your customers could fill out on your storefront. There are a few apps that can provide you with this feature. Be sure to check out; Survey + Poll, FormPopper and Form Builder by Arctic Grey. 

An alternative option would be to use an app to collect information from the customer on the product page. This might work the best for your business idea. I have a few apps in mind that could work with this method:
