Hello, does anyone know if there is any app able to replicate this website options?

 1) Choose a box size  (4 pcs)
2) Choose from a list of options with qty field and with qty limit related to box size of 4 pcs

For this, you could try the product option by Bold. The most important stuff is their Conditional Logic feature. If this app doesn't meet your need. You also want to hire a dev to add a custom code for your stores.
