Product subscription with option to pick multiple dates

I want to sell "Bouquet Subscriptions" to my clients.  I would like for them to be able to choose:

• how many weeks they would like to purchase to have a bouquet delivered/picked up
• choose 4 to 6 dates that they would like their bouquet delivered or picked up

I would be selling the bouquets from April through September and will have them available for most Tuesdays through the summer.  I would like an order form that allows them to pick the number of weeks they want to purchase, along with choosing multiple dates that they want the product delivered. I would also like to have all this information be available to me in an excel form or some other way, so I can access the orders placed and for which days I need to deliver or have ready for pickup.

Are there any apps out there that can do this?
