Subscription app for multiples of one product

Hi,
What I'm after is pretty simple I thought but I just cant find the right app. I have a website with only one product (a vitamin lasting 2 months per bottle) and I want a subscription app to be able to send 6 months worth of the vitamin out at a time, so 3 bottles at once.

Does anyone know of an app that allows for me to allocate an option for the 3 bottle subscription along with the one time payment?
