Subscription App to build your own subscription boxes?

I see there are many apps that enable a subscribe & save button on any items in your store, but thats not what im looking for. I want to build my own subscription boxes and set them up for choices such as monthly, six months and yearly subscriptions. Im looking to do something like this...

I think most of the subscriptions apps allow you to create a subscription plan Monthly, Weekly, etc with a discount. Similar to what you're looking for. You can check out this article for some suggestions.

Our app, BYOB allows customers to build their own boxes. The products are displayed on a custom template for selections. Currently, we also support integrations with subscription apps like Recharge.
