Third-party app that allows people to upload testimonials or reviews?

Is there a reliable third-party app that allows someone to upload testimonials? Some features that would be helpful would be:

• Front-end UI that allows a guest to upload a testimonial/review
• Ability (for admin) to manage testimonials (publish, unpublish, edit, etc.)
• Allow manual upload (for admin) either by html, csv, etc.
• Front-end UI that sorts testimonials and pagination. Since reviews or testimonials can vary in length by user, it will be great to choose how many to show at a given time. This helps us show these reviews without having to compromise the layout of a specific page where they will appear.

It will be also great if we could control whether to accept max characters, media types (photo limits and videos). If anyone has any insight, we would appreciate the info. It doesn't have to be a Shopify app and will be open to an embed type of option.
