Using Shopify for Donations - one time/recurring

Hello guys,

I am looking for a solution to create a great donation experience, where user have multiple choices:
- one time/recurring payment
- pick one of the amounts
- enter custom amount (both for one time and recurring)

The screenshot below illustrate it just fine. After clicking "continue" we would go straight to the checkout.

How would you tackle this challenge? Do you think it's even possible with a single app + own (simple;) scripts?
