What app can help build a customer return form?

Hello all,
I am looking for an app with which I can build a form.

In this form, customers should be able to enter which items they are returning and whether they would like to have a replacement product. In addition, one should be able to export all requests that come through the form. The last point is important.

Can anyone recommend me an app?

I recommend however, getting in touch with the app developers directly to confirm an apps capabilities and ensure it can achieve everything you're looking for. To contact an app developer, you can head to the Shopify App Store and select Support > Get Support.
