What app can manage multiple add on sections for a gifting product page?

Hi is there an app that would allow multiple add on sections on a product page (i.e. multiple steps with different layouts). We are a gifting site and we'd like the right-hand side of our product pages to show (depending on product type) :

1.  'add one more more product in the gift basket' (grid display for predefined products). 
2. 'select a card (drop down menu)
3. 'enter your greetings' (free text/ to & from fields; mandatory field with check box for possibility to leave blank)
4. 'enter date' (free text / calendar field)
5. Upload image (file/image uploader) - predefined rule/displayed only if specific product(S) selected)

I suspect this would require multiple separate apps to be installed. Please recommend which is the best option for each.

Thanks @Blair . What if my product was a giftpack with multiple products (each tracked in inventory) and each product has a variant I'd like the customer be able to adjust. Is there an app that manages this smoothly both on the frfont end and in admin?
From the admin stand point I am currently manually adjust inventory count of the individual products everytime a 'pack' is created/added in inventory.
