Which easy-to-setup app allows customers to upload files at checkout?

I need to find a file upload App that offers customers the option to upload a file at checkout. I can find many aps that let the customer upload a file from the product view but I think it would be best at checkout.

The files are artwork, so they will be graphics files.

Can anyone recommend an APP that will do this that's easy to set up.
