Which Product-Addon with Add To Cart Price connection

Can anyone tell me what app this store is using to show its product add-ons?

They're linked to the add to cart button and updates the price.

The reference site you shared have hardcoded the functionality. It can be similarly hardcoded in your site with similar layout.

You can also opt for an app but matching the same layout can be a limitation.

How does Shopify Theme updates work with hard coded functionality?

When shopify updates it theme, it updates theme features and code. So hard-coded functionality needs to be pushed to the mew updated theme. It depends on developer if he/she moves it for free of costs or not.
