Why won't product reviews display on my Refresh theme?

I am using the theme Refresh and i am unable to add the reviews to my shop

I'm sorry to hear that your reviews aren't showing up in your store as expected! In order for us to pinpoint exactly what the cause may be, and to see if we need some assistance here from the Live Support Team, I'm have a question:

• Are you seeing any types of error messages when logging in or using the Product Reviews app? 
• If you are seeing any errors, can you please share them with us here? Screenshots would also be incredibly helpful!
• Additionally, if you could provide a link to your store as well, that would be even better!
