How can I add in another text box saying 'Profession / Business' as shown in the picture below?

Can I ask a followup question? How are you planning to use this information once you have it? Are you planning on going in and manually adding the "profession/business" answers to individual customers in the notes section?
If you're looking for a more seamless way to add this information, a custom field on a customer registration page would be ideal (given the amount of information I have).
