Adding an image to packing slips

Trying to add an image to the bottom of our packing slip, before the store name. I have inserted this code:

at various places and it never shows up. 

What am I doing wrong?

Hi Again! can you tell me what the difference is? it looks the same to me except for the hard return after img.

Yes, tags in html only support ", so when you add by “, it will not work.
