Adding custom text per product variant

Hi,

 

I'm trying to add custom text for a variant option. 

 

For example this business sell cakes and I have a product variant called "Additions" with two options being "None" and "Birthday Plaque." We want the customer to be able to write free text if birthday plaque option is selected (as it charges the customer extra) how do I do this? 

And then the other aspect is this showing up on the New Order sheet for the kitchen team (I'm sure this is somewhat straightforward as a HTML input)
