Cant edit design of review form with Product Reviews application.

Hi, i would like to ask you if its possible to customize review form in some css or liquid file? I have imported it in theme through snippets, but i cant customize it as i need. I tried to find any liquid file, but without success. The specific problem is the required modification of the form according to the design in figma, it is about adding inputs, changing the layout and adding a select for a specific product, so that it is possible to add a review always for a certain product. It's a solution I adopted from someone else and I'm having a problem with it. Is this even possible using this app? Thanks in advance for your answer. I am supposted to use this app, because of functionality. But now i  have struggle with customizing their form...

You can customize but product review app by Shopify does not allow to upload image.

I dont need to upload image, i would like to add some fields and customize form layout, but I can't find either liquid or css file of form created by this app...
