Contact Us form in collapsable content

Hi All

im looking to add a contact us form into a collapsable content row.

I did get a previous answer but I can't work out how to add it in, if any one can help that would be great.

Im using dawn theme 11

This need to do custom code in your theme file to ensure the contact us form should show when you clock on the collapsible content.
