Creating a shopify checkout section on a page

Hello, I have removed the checkout icon from my website. I've made a "request service page." I want it to have a form you can fill out, followed by a submit button, or followed by fields that the user can use to immediately enter there credit card and billing information.

To put it simply.. I have already added the formify custom checkout app. How do I make one of my shopify pages go directly to checkout? Thanks!
