Customizable Submission form

Hi I would like to know if anyone could help do the submission form, how to customize it using third-party app for shopify.

This is the design I was thinking using formspree.io:

I know there's a lot of third-party app for forms, my idea comes with this type of form:

basically, you doing want build a form based on third-party form, and then you want to that form can custom background, color, and field depend your bespoke, right?

About the style you can achieve by make style, and add more setting depend to your bespoke. Beside if you just want a form that provide common fields as like name, email, file.., not special customize, you can refer form Shopify here
