Embedded Form not popping up in password page

Hi,

 

I tried to insert my Klaviyo embedded form to my Symmetry theme password page but it doesn't seem to pop up. But when I inserted it on Baseline's theme, it popped up. What could be the issue? I would appreciate any help.

You did not add form ID, you added HTML code.

Appreciate the response. Just changed it to my Klaviyo form ID and still doesn't work.

It works now! I just noticed my Klaviyo isn't integrated with Shopify. I didn't know it unlinked since my Klaviyo account was disabled recently 😂 Thank you though!
