Embedding the Shopify Contact Form into a Zipify Page using HTML

I am trying to put the Shopify Contact Us Form into a Zipify Pages page so we can add more information like our FAQ's etc before and after the form and to maintain consistency since all of our other pages are built on Zipify.  I am having trouble doing this.  Anyone have any suggestions or can help?

Did you ever figure this out?  I would like my contact form in zipify pages too, but I'm having the same problem.
