Help Adding Form to Checkout Page

Hi all!

I currently have a form that populates on all my items on my website that customers have to fill out prio to checkout. However, this has been missed by some causing a few issues as I was not able to make it a required form prior to checkout. 

Happy to work with someone who can add a similar form on our checkout page that must be filled out prior to paying.

That's definitely an issue that can cause problems with your checkout process. I think I might have a solution that would work for you. It would require some custom coding, but it's a relatively simple change that would help ensure that customers fill out the form prior to checkout. Can I ask what platform your website is built on?
