Help learning image popup.

I was modifying a theme on my website, usdentalinc.com, to add the ability to make a popup be an image instead of a newsletter signup.

I found an answer here:

I simply pasted in the code which works great, however the options to not make the popup show up on every page does not work. 

EDIT: I am running ware-house theme V2.8.4

I am also looking for a way to have the pop up takes users to a collection when clicked on.
