How can i edit Popup on my website from add to cart button

I have a store on shopify, Theme was Dawn, and store name as Mediland Pharmacy. This is the url- https://medilandpharmacy.co.uk/

Here I attached a screenshot that had come on my product page while clicking on add to cart button.

I would like to add more fields to here, like this- 

I tried to add code on theme file, but i have no experience on liquid coding and also i don't know where this are on liquid file.

Welcome to Shopify Community.

It is better to either look for a Shopify app or hire a Shopify Developer for these kinds of features as simple copy/paste solution may not work for this.

In fact, you might need to save these info on the admin for references purpose as well & there will be some good amount of modification to achieve this.
