Hi, hope to get someone to help on my contact us form. I don't know how to get the form functioning. i.e. set up a box form for customer to fill up details, how to create a send/submit button at bottom for customer to submit once he has filled up the details.

It's better to follow what @KetanKumar said: use the theme's contact form.

To do so:

Go to Online Store -> Themes -> Theme Customizer
Within the theme editor, click on the Contact Us link, on your top nav.
Now click "Add section" on the left menu and select "Contact Form". Then save it.
Now go to Pages -> Contact Us and remove the contact form using the page editor.

sorry but your contact page doesn't shopify default contact page so can you please use theme contact page code
