How should I add a text box where customers can request a product to be added to my website?

I want to give my customers the option to request products that they- may be looking for that we aren't offering just yet. I'm unsure of how to add a box to submit a request and it goes directly to our team email address.

 

Please help!

Two ways to do it:
1) Customize the theme code.

2) Use an app ( using app you have to change the button text and it will work as you need )

Hi. I was also wondering about how to add a space for customers to send through suggestions. I just logged into my store's dashboard and used the Sidekick (The face icon for help and asking questions, which is on the top right of the screen, near your store name where you log out.) to ask how I achieve that. I was already in the Pages section, and the Sidekick offered to create a Customer Suggestions page, complete with submission form for me. (Submissions get sent to your store's email address and they're tagged as 'suggestion'.) I asked it to please help me to create the page, previewed it, made a few adjustments to the description and saved it. Then I went to my content and menus (under Online Store and Navigation for some themes, I believe) and added the page to the home menu, dragged it to the spot I wanted, and when I viewed my online store, I had a Customer Suggestions page. Maybe that could work? (Hope this helps.)
