I am using Motion theme.

I want to sell a customized item, and it needs a survey to understand what customer needs. In my home screen, there is a "shop now" button. I want to have the "shop now" side button to a multiple page survey. The questions may include customer's name, and include the name in next page. It needs to include progress bar.
