Order Number Access To PDF - Custom Code

Hello,

I am trying to make a page that includes the proprietary installation instructions for my products but want to design a page where customers enter their order number or basic personal information as a 'passcode' to access the manual. 

Essentially, I am seeking code for a page that is titled 'Installation Manual' and requires customers to enter either their order number or last name included on the order number to receive access to the full page that exhibits a PDF download and installation video guide.

Hello! Yes all the products share one installation manual. In the future I may add one to two more for specific products but for now it will be a singular manual.

The most easiest way I propose is to restrict access to the 'installation manual' page, making it available exclusively to users who have logged in and placed an order.

Add a download link point to the pdf on the page where customers view their orders.
